authorize and contract with M J QUINLAN INSPECTION SERVICE to perform the following
services:

The claimant is:
I/We agree to pay, M J QUINLAN INSPECTION
SERVICE for work performed and billed. Signatory to this authorization and contract is not
with standing of relationship with third parties, contingency arrangements, subrogation,
etc. As a convenience, M J QUINLAN INSPECTION SERVICE may agree to prepare separate
billing for an attorney taking Consultant's discovery deposition, but the responsibility
for payment remains that of the under signed. Failure to include a chargeable item in one
billing shall not constitute a waiver of the right to assess the charges in a subsequent
billing. I/We agree and understand that M J QUINLAN INSPECTION SERVICE invoices are due
and payable either net on the day of the inspection or with prior arrangements within ten
(10) days of presentation, and that the maximum legal rate of interest will be charged on
all, invoices past due.
Should any arbitration or action at law or in equity be commenced by M J QUINLAN
INSPECTION SERVICE against owner, growing out of or in any way connected with this
agreement to enforce or interpret its items and provisions, the prevailing party shall be
entitled to recover reasonable attorney's fees and to be fixed by the court in addition to
other remedies to which it may be entitled.
